This post is specifically for same-sex couples and single mothers by choice who are wondering how acupuncture and integrative fertility care fits into their path. Here’s our approach.
For same-sex female couples and single women using donor sperm
For female couples and single women using donor sperm through IUI or IVF, the fertility acupuncture protocol is essentially the same as for any woman trying to conceive with a few important differences in how we think about the clinical picture.
IUI with donor sperm: optimizing for success
IUI with donor sperm is one of the most common paths for same-sex female couples and single women. The per-cycle success rates are similar to IUI with partner sperm: 10–20% per cycle for women under 35. The same factors that limit success apply: egg quality, uterine receptivity, ovulation timing, and the quality of the donor sperm.
What we do for IUI patients:
- Egg quality optimization — starting 2–3 months before the first IUI cycle with the full mitochondrial supplement protocol and weekly acupuncture
- Uterine lining preparation — specific acupuncture protocols focused on uterine blood flow and lining quality
- Cycle regulation — for women with irregular cycles, we work to establish consistent, well-defined cycles before beginning treatment
- Ovulation support — timing acupuncture treatment to support the LH surge and follicular rupture
- Donor sperm considerations — we review the sperm analysis of your chosen donor and discuss any parameters worth noting
IVF with donor sperm: the same protocol, optimized
For female couples and single women pursuing IVF with donor sperm, the protocol follows our standard IVF support approach beginning 2–3 months before retrieval with intensive egg quality optimization. The donor sperm component is typically handled by your fertility clinic, but we review the analysis alongside you and can discuss whether additional sperm parameters are worth evaluating.
There is no one way to build a family and we support all the pathways it can take to build a family. It takes a village to raise a baby, and we think it takes a village to make one too.
Reciprocal IVF: supporting both partners
Reciprocal IVF, where one partner provides the eggs and the other carries the pregnancy, is a beautiful option for female couples who want both partners to be biologically involved in the pregnancy. It’s increasingly offered by fertility clinics in San Diego.
In reciprocal IVF, each partner has a distinct clinical role:
The egg provider partner: Undergoes ovarian stimulation and retrieval. The protocol for her is identical to our standard IVF egg quality optimization: acupuncture starting 2–3 months before retrieval, the full mitochondrial supplement protocol, and phase-specific treatment during stimulation.
The carrying partner: Prepares her uterus to receive the transferred embryo. The protocol for her focuses on uterine receptivity: lining quality, uterine blood flow, immune environment preparation, and transfer day support.
We love reciprocal IVF because it gives us the opportunity to work intensively with both partners simultaneously each on her own protocol, each supporting the other. It’s one of the most collaborative clinical experiences we have.
For same-sex male couples using a gestational carrier
For male couples working with a gestational carrier, the clinical focus is on sperm quality because the embryo quality is almost entirely determined by the sperm contributing partner or partners.
Our protocol for the sperm-contributing partner:
- Full sperm quality assessment — semen analysis plus DNA fragmentation testing, which we recommend for all male partners
- 12-week acupuncture protocol — targeting count, motility, morphology, and DNA fragmentation
- The full antioxidant supplement protocol — CoQ10, vitamin C, vitamin E, zinc, selenium, NAC, lycopene
- Lifestyle optimization — heat avoidance, alcohol reduction, sleep support
For the gestational carrier, if she is working with a fertility clinic in San Diego and is open to acupuncture support, we offer uterine preparation and transfer support protocols. We coordinate with the clinic and the carrier’s own healthcare providers.
On-site embryo transfer support at the IVF clinic helps create an environment of physical and emotional support for both the gestational carrier and intended parents. When we provide that day-of support, we function as your “embryo transfer doula”.
The emotional landscape: what’s different
The clinical protocols for same-sex couples and single mothers by choice are largely the same as for heterosexual couples. What is often different is the emotional landscape.
Many of our LGBTQ patients and single mothers by choice have been navigating a fertility system that was not originally designed with them in mind. Forms that assume a partner of a different sex. Waiting rooms full of heterosexual couples. Practitioners who ask questions that reveal they’ve never thought about their situation. We see this, and we take it seriously.

Frequently asked questions
Do I need a referral from a fertility clinic to start acupuncture?
No referral needed. Many of our patients come to us before they begin working with a fertility clinic, or while they’re in the process of choosing one. We can help you think through the process, understand your options, and begin optimizing before the first procedure.
We’re using a known donor. Does that change your protocol?
Not significantly. If the known donor is willing to complete a semen analysis and DNA fragmentation test, we can review those results with you and discuss whether there’s anything worth optimizing. Beyond that, the protocol for the recipient partner is the same.
Can you support my gestational carrier even if she’s not in San Diego?
We offer virtual coaching for gestational carriers outside San Diego. We work with her local acupuncturist when possible, provide the supplement and nutritional protocol, and offer virtual support throughout the transfer preparation and two-week wait.
