You’ve done everything right. Your embryo was chromosomally normal. Your lining was adequate. Your protocol was textbook. And yet, it didn’t implant. This is one of the most devastating and most poorly explained experiences in IVF, and it points increasingly toward something that standard fertility medicine is only beginning to address systematically: immune implantation failure.

At Aphrodite Fertility Acupuncture, this is one of the most important areas of our clinical work. Here is what immune implantation failure actually is, how to investigate it, and what can be done.

What immune implantation failure is

Successful implantation requires a delicate immune negotiation. The embryo is genetically foreign to the mother (it contains the father’s DNA) and the immune system must be actively suppressed in the uterine environment to allow it to implant and grow. When this immune tolerance is disrupted, the body can reject even a healthy, chromosomally normal embryo.

The primary immune mechanisms involved in implantation failure:

  • Elevated uterine natural killer (uNK) cell activity — natural killer cells in the uterine lining play a complex role in implantation. When their activity is elevated or dysregulated, they can attack the implanting embryo rather than support it
  • Cytokine imbalance — the ratio of pro-inflammatory to anti-inflammatory cytokines in the uterine environment affects whether implantation succeeds
  • Antiphospholipid antibodies — these autoantibodies cause small blood clots in the placental vessels, interfering with early placentation
  • HLA-DQ alpha compatibility — when both partners share similar HLA antigens, the embryo may not be recognized as sufficiently ‘foreign’ to trigger the protective immune response that normally supports implantation
  • Thyroid autoimmunity — elevated TPO antibodies are associated with increased implantation failure and early pregnancy loss, even when TSH is normal

Investigating immune implantation failure

Standard fertility workups don’t include immune testing. For patients with recurrent implantation failure, defined as two or more failed transfers with good-quality embryos, the following investigations are worth pursuing:

  • Uterine natural killer cell testing — peripheral blood NK cell testing (CD56+) as a proxy for uterine NK activity
  • Antiphospholipid antibody panel — lupus anticoagulant, anticardiolipin antibodies, anti-beta2 glycoprotein
  • Thyroid antibody testing — TPO and anti-thyroglobulin, even if TSH is normal
  • Cytokine profiling — available through specialized reproductive immunology labs
  • HLA typing — both partners, if other causes have been ruled out
  • ERA (endometrial receptivity analysis) — to distinguish implantation window timing from immune issues
  • ALICE testing — to rule out chronic endometritis as a contributing factor

How acupuncture addresses immune implantation failure

Systemic inflammation drives immune dysregulation. The dietary and supplement protocol for immune implantation failure is intensively anti-inflammatory:

  • Fish oil (EPA/DHA, 3–4g daily) — potent anti-inflammatory, directly reduces NK cell activity
  • Vitamin D (optimizing to 60–80 ng/mL) — vitamin D deficiency is strongly associated with immune dysregulation and implantation failure

  • Intralipid infusions — fatty acid emulsions that reduce NK cell activity, used before and during IVF cycles
  • Low-dose steroids (prednisone) — suppress immune activity during implantation window
  • Low-dose aspirin — for antiphospholipid syndrome and blood flow support
  • IVIG (intravenous immunoglobulin) — for severe cases with confirmed immune dysregulation
  • Progesterone support — progesterone has direct immune-modulating effects and supports the anti-inflammatory environment of implantation

Frequently asked questions

How do I know if immune issues are causing my implantation failures?

The most telling sign is multiple failed transfers with chromosomally normal embryos and no other identified cause. Other indicators include a history of autoimmune conditions, recurrent miscarriage, thyroid antibodies, or a family history of autoimmune disease. If this describes your situation, push for immune testing before your next transfer.

Can I be tested for immune issues before I start IVF?

Yes — and we recommend it for any patient with a history of autoimmune conditions, known thyroid antibodies, or a previous pregnancy loss. Getting this information before the first transfer allows you to address immune factors proactively rather than reactively.

Does acupuncture alone address immune implantation failure?

Acupuncture is a meaningful part of the protocol but not the complete answer for confirmed immune implantation failure. It works best as part of a comprehensive approach that includes medical immune management (through your RE or reproductive immunologist), anti-inflammatory nutrition, and targeted supplementation. We coordinate with your medical team.
