If you’re working with a fertility clinic in San Diego, you’re likely navigating the decision between IUI and IVF or being walked through a protocol that starts with one and may progress to the other. What most REs don’t have time to tell you is how much the outcome of either procedure depends on what you’re doing in the weeks and months leading up to it.

Acupuncture is not a replacement for IUI or IVF. It’s the preparation and support that makes them work better. Here’s exactly how.

IUI: what it is and what limits it

Intrauterine insemination (IUI) places washed, concentrated sperm directly into the uterus around the time of ovulation, bypassing the cervix and reducing the distance sperm need to travel. It’s less invasive and less expensive than IVF and is often the first assisted reproductive technology recommended for couples with unexplained infertility, mild male factor, or cervical factor.

The typical success rate per IUI cycle is 10–20% for women under 35 with unexplained infertility and lower for older women or those with additional factors. Most clinics recommend 3–4 IUI attempts before moving to IVF.

What limits IUI success:

  • Egg quality — IUI doesn’t influence the quality of the egg being ovulated. A poor-quality egg produces a poor-quality embryo regardless of how the sperm gets there
  • Uterine receptivity — the uterine environment at the time of the IUI matters enormously for implantation
  • Timing — IUI is exquisitely timing-dependent. Triggering at the right moment is critical
  • Sperm quality — even washed, the quality of the sperm matters. DNA fragmentation affects IUI outcomes as much as IVF outcomes

How acupuncture supports IUI

For IUI patients, acupuncture addresses exactly the factors that limit success:

Egg quality optimization. Starting acupuncture 2–3 months before IUI cycles allows us to meaningfully improve egg quality through the mitochondrial pathway. The egg ovulated in an IUI cycle is the primary determinant of whether conception occurs and whether the pregnancy sustains.

Uterine lining preparation. We focus on uterine blood flow and lining quality throughout the cycle. A well-vascularized, trilaminar lining at the time of IUI is just as important as in IVF and receives far less attention in IUI cycles.

Ovulation support. For natural and lightly stimulated IUI cycles, acupuncture supports the LH surge and follicular rupture. We time a treatment in the 24–48 hours before the planned IUI to optimize the uterine environment at the time of insemination.

Stress reduction. IUI cycles carry their own particular anxiety, particularly cycles 2 and 3 after previous failures. The cortisol load of repeated disappointment directly suppresses the hormones needed for success. Acupuncture addresses this physiologically, not just emotionally.

In an IUI cycle, acupuncture treatments not only supports follicular development and robust ovulation, which in turn promotes strong hormone levels, but also thickens and nourishes the uterine lining.

IVF: what it is and what limits it

In vitro fertilization retrieves multiple eggs, fertilizes them in the lab, and transfers one or more resulting embryos into the uterus. It offers more control, more visibility into what’s happening (fertilization rates, embryo quality, genetic status), and higher per-cycle success rates than IUI.

The factors that limit IVF success:

  • Egg quality — poor egg quality produces poor blastocysts. This is the primary driver of failed retrievals and poor-quality embryo cohorts
  • Uterine receptivity — even perfect embryos fail to implant in an unreceptive uterus
  • Ovarian response — poor response to stimulation medications reduces the number of eggs available
  • Embryo quality — determined largely by egg quality and sperm DNA integrity
  • Implantation factors — immune environment, lining quality, timing

How acupuncture supports IVF

The 90-day egg maturation window is the critical intervention point, and the vast majority of IVF patients miss it entirely.

What acupuncture specifically does for IVF outcomes:

  • Improves egg quality in the 90 days before retrieval — the most important intervention
  • Supports ovarian response to stimulation medications — more follicles recruited, better synchronization
  • Reduces OHSS risk in high-responders
  • Improves uterine lining for fresh and frozen transfers
  • Transfer day treatment — pre and post transfer — the most researched application
  • Two-week wait support — progesterone, uterine calm, nervous system regulation

  • Egg quality optimization in the months before treatment
  • Male factor optimization beyond the semen analysis
  • Nutritional and supplement protocols that support treatment outcomes
  • The emotional and psychological dimensions of fertility treatment
  • Lifestyle factors — sleep, stress, heat avoidance, dietary changes — that influence sperm and egg quality

Frequently asked questions

When should I start acupuncture before IUI or IVF?

Before both — and as early as possible. For IUI cycles, starting 2–3 months before your first planned IUI allows us to optimize egg quality and uterine receptivity meaningfully. For IVF, 3 months before retrieval is the target. Even starting 4–6 weeks before is significantly better than starting at the transfer.

Should I do acupuncture on the day of my IUI?

We recommend treatment in the 24–48 hours before IUI to optimize uterine blood flow and lining receptivity at the time of insemination. Some patients also do a brief treatment on the day of the IUI itself. We tailor this to each patient’s schedule and clinical picture.

My RE has recommended 3 IUIs before IVF. How should I use that time?

This is actually a valuable window. Three IUI cycles at 4–6 weeks each gives you approximately 3–4 months of preparation time. We’d use this entire period for intensive egg quality optimization, so that if you do move to IVF, you’re retrieving the best-quality eggs possible rather than starting IVF cold.

Does acupuncture change the decision between IUI and IVF?

Not directly, that’s a medical decision based on your clinical picture. But it does change the success profile of whichever path you choose. Women who commit to 90 days of optimization before IVF often produce better embryo cohorts, see better lining preparation, and have better transfer outcomes. This can meaningfully change how many cycles are needed.
