First — What Is Actually Happening in Your Body Right Now?
After ovulation, your body leaves behind something called the corpus luteum — the little sac that remains after your egg releases. Its entire job is to produce progesterone, which in urine we measure as PdG (Pregnanediol Glucuronide).
PdG needs to rise and stay elevated throughout your luteal phase to support implantation. If it’s doing that, your body is on track. If it drops too early or never rises adequately, that’s genuinely important clinical information – not just a feeling, not bad luck, actual data you can bring to your care team.
Here’s the problem: most women go into the TWW with zero information about what their progesterone is doing. So you’re left reading sore breasts and a heightened sense of smell as either proof of pregnancy or signs of impending heartbreak. Neither of those is a real data point. Both of them will send you down a rabbit hole at 10pm.
PdG monitoring changes that completely. When you know your hormones are doing what they’re supposed to, you have something solid to stand on — even in the uncertainty.
The TWW Protocol We Give Our Patients
Here’s exactly what we walk our patients through when they’re heading into their two-week wait:
Step 1: Confirm ovulation before the wait even starts.

A positive OPK is not confirmation. It tells you your LH surged, but not that your body followed through. Confirmed ovulation means your LH surged AND your PdG rose afterward. Step 2: Start monitoring PdG at 3 DPO.

Keep a simple daily note — not an anxiety journal, just a quick check-in. Sleep quality, energy, any spotting, mood. When you pair your subjective experience with objective hormone data over a few cycles, you start to see your actual patterns. You stop interpreting every twinge as a verdict and start genuinely understanding your body. That’s a shift that changes everything.
Step 4: Come in for acupuncture — especially in the second week.

But there is a real difference between feeling your feelings and white-knuckling through twelve days of compulsive Googling at midnight. Data gives you a foothold.
