Hyperprolactinemia is a common and often overlooked cause of ovulatory dysfunction. In fertility care, it belongs on the differential any time a patient presents with irregular cycles, amenorrhea, galactorrhea, low luteal progesterone, or unexplained anovulation.

Pathophysiology

- Medications, especially dopamine antagonists and some psychotropic agents.
- Pituitary adenoma or other sellar pathology.
- Other endocrine or systemic conditions that disrupt pituitary signaling.
If the prolactin elevation feels mild, repeat testing under ideal conditions may help. However, if levels stay elevated, further workup becomes important, especially when symptoms align with ovulatory dysfunction.
Testing Strategy
A practical fertility workup may include:
- Serum prolactin, repeated if mildly elevated.
- TSH, free T4, and thyroid antibodies when indicated.
- Cycle-day-appropriate FSH, LH, and estradiol.
- Mid-luteal progesterone, ideally 7 days post ovulation.
- Pituitary MRI when prolactin remains elevated or when neurologic symptoms are present.
For cycle tracking, timing matters. In patients with irregular cycles or PCOS patterns, hormone monitoring tools can help identify the true ovulatory window before we interpret luteal progesterone.
Integrative Support
Acupuncture can support HPO axis regulation, reduce sympathetic tone, and improve the patient’s overall resilience while the medical team addresses the underlying cause. In practice, this works well alongside sleep support, nourishment, blood sugar stability, and weekly consistency of care.
Helpful adjuncts may include:

- Nutrient-dense meals with adequate protein and calories.
- Gentle, consistent movement instead of overtraining.
- Collaborative referral when medication or imaging becomes necessary.
We should avoid oversimplifying the case as “stress.” Stress can affect the body, but hyperprolactinemia is a medical issue that deserves proper evaluation and treatment.

Referral Thresholds
Refer or coordinate with an MD or DO when:
- Prolactin remains elevated on repeat testing.
- Galactorrhea appears.
- Amenorrhea persists.
- Headaches or visual changes show up.
- Thyroid testing is abnormal.
- The patient needs medication management or imaging.
Bromocriptine or cabergoline may help in medical management of prolactinoma or persistent hyperprolactinemia, depending on the broader clinical picture.
